To create a fairly easy contact manager which you could probably find a prewritten script to very easily along with three other very simple forms our church uses and are filling out manually at this time. I want a clean looking front page with four icons describing each form which is nothing more than a hyper link to their respective form. All four are to “share” the same customer member data field so that all members / visitors / or whomever entered into the system from the church employee should be shared by one single database as a center hub so that each form will "share" the contact info! So, if you filled out a "Alms Request Form" (see attachment) already and then they contacted someone else in the office and needed to be documented in the "Contact Manager Database" (a script needs to be found for this) or maybe now they want to fill out a " Biblical Guidance Form " (see attachment), all their pertinent information will automatically show up filling in the majority of the new paper work cutting way back on unnecessary repetitive work making the employees more efficient.
Then from the admin side with limited management (there needs to be levels of security on who gets to log in and what they get to edit or see as designated as top admin) will require independent login names and passwords as issued within the admin and I want in the admin only to be viewed by the top admin to show a time and date stamp of each employee(s) or management use - logging all activity viewed by upper management and time spent in the admin between login and log out. A simple report of these activities should be made available for print out on any individual in the system whether it be management activities or time lines spent logged into the admin or if admin wants to see a report of all activity from any given form which can be selected in the admin as a query request which will show all contact information or data entered on the “contact” for the last year or specific date to specific date requested.
From the front end, simply typing in someones last name, first name, phone number, or any other data (unique automatic member numbers should generate a list of past conversations from any form or document here. A unique member number is to be used from the entire phone number like 909-590-4227 would become automatically member no. 9095904227 and pull up their info everytime. This should be made editable in the admin in case a phone number is updated at a later time or if someone simply edits the phone number, then their member number would automatically change as well along with it.
p.s. Keep the forms 2-4 below in the exact same appearance they are already in. Type the front end to look the same so there is no learning curve for the employees and they will see something they are already familiar with. The only difference is instead of writing the info in manually, they will be able to type it in now and it will be stored in the database.
This is just a quick form filler and contact manager which will contain the following…
1. Nice Contact Manager with note reminders of previous conversations to act as a reminder of previous conversations when someone calls. (look for a free and nice script online)
2. Alms Request Form (see attachment)
3. Biblical Guidance Form (see attachment)
4. Benevolence Request Form (see attachment)
WHAT I EXPECT FROM A PROVIDER:
As you can see by my profile and feedback I do this full time. I’ve been a USA and Canada based project manager for many years now and trusted. If you win the bid and yes of course I’ll be looking for the best price, I will post the money into escrow or if you require another form of payment such as Xoom, PayPal, or MoneyBookers, I have accounts with these. Because I’ve been burned and lost customers as a result over the years, no money will be released until the job is 100% complete to my customers satisfaction so don’t bid the job and ask for money if you aren’t finished with the job. I have no problem securing the money for you though such as with GAF so you can trust me or you can just look at our feedback because we pay very fast and immediately as soon as the job is completed as described here in the job description which I’m always very clear about. If for some reason my customer asks for something in addition and not already here in the job description, I expect there would be an additional fee unless it’s very minor so I’m not the type to take advantage of a programmer and keep adding to a job. The only thing I expect from a programmer is that you do the job for the money you bid without trying to ask for more money half way through the job (this really get’s me upset) and within the time frame promised because if you don’t, it makes me look like a very unprofessional person in front of my customer and I can lose my customer. Just be a man or woman of your word and don’t say later on you didn’t know it was going to be so much more work than you thought. If you don’t know what it’ll take to do the job than don’t bid! I don’t need the headache.
If you do an exceptional job without giving me any stress as asked here in the job description, I have about 8 jobs a week plus and actually always overwhelmed with work and will give you more and more work which for my favored programmers has become many many years of repeat business and a great business relationship!
Give the best bid price with an accurate finish date only if you are a professional in the field listed here to do the job which this is a simple script install and database build and you can be looking at a long term relationship. Please ask any questions you have before bidding to clarify any questions in advance.
P.S. Send me a link to the Contact Manager Script you plan on using ahead of time so I can review it first before I award the job. I’m also looking at awarding this job this week!
