Two forms - Php and Mailchimp expert. Mailchimp single opt form.

Hello Freelancers, I'm looking for a PHP and MailChimp expert to develop two forms and set them up with my website and my account on MailChimp. The first form is simple and should only contain: name, email, phone and post.( side bar of all pages of the site ) The second form presents some more data and options checkbox.( one especial page) All form fields should be mandatory, except the message field. Each form must send their data to two different lists on mailchimp. Must be a single opt form. I wish it were not necessary to subscriber needs to confirm your subscription. These are forms for my new site that is still on my PC. The project includes attaching the form on their sites and it fits the style of my site. The orms are in Portuguese, but no problem, I'll help you.
