Team Assessment

This is a multi-input, web-based team assessment project. Any number of users can register from a "team" online. Each is given a unique alphanumeric ID. A separate page allows someone from team to enter all ID's for team members, then a Team ID is created. Each team member, on a separate web page, enters their individual ID and the Team ID, and completes an assessment that uses 5-option graphically clickable answers for each of about 40+ questions. The questions are divided into sections so the assessment taker can review their answers before moving to a new section. Maybe 8 sections or so. When every Team member has completed the assessment, the system processes the averages for the team and creates a Team report, which includes: - average score for each question - the national mean (we provide)(fixed number) - number of people who selected each of 5 or so answers. - nice looking graphical representation of data - probably bar charts. Report is automatically emailed to the person who has selected "yes" to "Are you the designated Team leader?" The system should store the raw data for later retrieval by Team ID. ## Deliverables Some thoughts I've had about how it works: When User IDs are entered into the Team Member page, the system comes back and provides the names of each of the Team members per those IDs. Then, the person entering the User IDs confirms/verifies with click of button that these are all the right people. If not, they are taken back to the entry page to make adjustments. When they verify "Team Members are Correct", the system provides a Team ID. There is a page where each person on the team will go to begin the assessment. They enter the Team ID and their User ID. The system identifies the Team Name and the User Name. When they verify that is correct, they are taken into the assessment. Each page of the assessment has a number of questions, with 5 0r 6(I'll have to verify) answers. They will click on the "button" representing their answer for each question. At the end of that page of questions, they are asked to review their questions before going to the next section. They click on button to go to next section. Once they go to next section, they cannot go back to a previous section. They proceed through each section until all sections are completed. At end of last section, they click on Go to Next Section, and hey are taken to a "thank you" page. The system says, "Congratulations! You've completed the assessment. Your designated Team leader will get the Team report when all Team members have completed the assessment. Thank you for your valued input!" Once all Team members have completed the assessment, the data analysis (simple averages) is completed and the report is available by the Team ID, and notification is sent to the "Designated Team leader." The report is available on a page by entering Team ID, and they can enter email addresses for where to send it by pdf. Report includes data provided graphically (bar charts), as well as numeric data. Based on some questions, here is some additional information: The answers to 40 questions are all: -True -More True Than False -More False Than True -False Those would be assigned values of 4, 3, 2, 1 respectively. The Team score would simply be the average of all team members, EXCEPT the Designated Team Leader. The DTL score would be compared to the Team score and the National Mean in the bar charts. 2. Clean, simple, professional design. Nothing fancy. I imagine to the right of the question would be the four answers and a radio button for each. When the appropriate button is clicked on, I would expect a check mark to appear there. [login to view URL], I want the report to show bar graphs for each question showing the national mean bar vs the team average bar vs the Designated Team Leader bar, with the actual number showing as well. 4. The purpose is to have a number of people on a team or organization to be able to anonymously share how they see the team's effectiveness in 8 areas, including leadership. It's like a 360 degree assessment for the team. 5. I also want it to have the ability to have 6 open-ended questions for which people type out their perspective free form. Thank you for your efforts in your bidding process. Thanks to your excellent questions, I would like to communicate some additional information about expectations. I have a sample of a couple of pages of a report that we will need to have. Can someone tell me how to post a file and I'll do so? We would welcome the report to look even better than the examples we'll provide that were just done in Excel I believe. The items to be calculated for the report and to be shown in a bar graph will include: [login to view URL] leader score vs average team member score for each of the approximately 48 4-answer-choice questions so it can easily(graphically) be seen where a particular team is strong, average or weak. 2. for each of the 8 topic areas that make up the 48 questions, the mean average for that topic compared to the national mean which we will provide. In other words the mean average score for a particular team for each of the 8 topic areas compared to the national mean for those 8. 3. a simple spreadsheet report of all people who have taken the assessment for each question showing numbers of people who marked each of the 4 answers, means and some other calculations that we will provide (simple statistical). 4. Reports above should be available as a pdf file. As people register for the assessment, their name and email address should be sent to our email database in a way that we can respond with a request to double optin to our subscriber list. I would ask that the reports be simple but professional looking. I would also ask that the web pages where they take the assessments be very professional. I'm imagining nice radio buttons that change their look for mouse over and checked or filled in when selected. I would ask that the first page where they register look consistent graphically with our website where it will be.
