Swish/Flash email form

I am looking for a flash email form. The site is under construction and has been built with Swish. I have created the form and a php script that doesn't seem to work. I know php is working on my server because I uploaded a test php file and it works fine.
So basically I am looking for somebody to get my email form to actually work. I can provide the swish file which I believe can be imported into flash. I can also provide the php script, although the solution doesn't have to be php.

I have programmed the non working php script to echo an error and the email address when it is submitted so that people realize there is an error.
Lastly the form has to fit into the current design.
