plugin/tool to read and display the parameters of a text element in an html form

I require a plugin or tool to be created which does the following: I need to collect similar data from multiple web forms. (1) When the mouse is hovering (or clicked) over a form on the page, your tool/plugin should display the form action, form method ( get or post), form_Url . (2) We basically are going to some forms used to submit data, and we want to note attributes of each text box(or radio button or other box which is there in the form). We will maintain a list of such parameters, eg "user_id" is parameter for user ID, "email_ID" is paramter for user's email address and so on. There can be any number of parameters which are already listed. When the mouse is hovering over a field/box in the html form , then field type (eg text box, password box, list box, check box, radio button etc) , name of this field (in source code there will be a name assigned to this field), min length, max length, and whether the parameter is mandatory or not are displayed. Also, the user can select a parameter from the list of parameters (eg if the field over which mouse is hovering is for User id, then the person can select this field as "user_id" (User_ID is already defined in the list of parameters). Also, if a new field is found (which is not listed in the list of parameters) then user can give a new name and description to this field, and add it to list of parameters. Now when user goes back to the new field, he can assign the name of the new parameter(as now noted in list of parameters) to this new field and store other attributes. (3) When mouse is hovering over captcha, the tool should be able to extract the name of file which is used to generate the captcha. Pls look at attached file...
