Hello: I have a checklist set up as a HTML form that I need to process on a daily basis. Ideally the script will work in conjunction with a HTML form that is set up as a daily checklist. The object of the checklist is to have users go through a list of daily responsibilities, and at the end of the day the user submits the form which will be sent to a supervisor via e-mail.
The following process should happen when the user submits the page:
-The user is redirected to a "thank you" URL that I have already published and hosted on a server.
-An e-mail is generated to one or more recipients with the following information:
-return email address of sender
-remote host name
-date and time sent
-list of checkboxes checked
-list of checkboxes not checked
The goal is to have the supervisor see what tasks were done for the day and what tasks were not accomplished to provide accountability.
The information needs to be presented in an easy to read format, as the information may need to be accessed by management.
In addition, I would like the ability to eventually submit the form data to a database, spreadsheet or report to be stored on a server. The only databases supported in the environment that I am aware of are DB2 and Sybase, although I might be able to get an Access or SQL one working as well. Unfortunately the users have no experience with database administration so whatever will be easiest to use and maintain would be preferred.
However, **the database function is not the priority at this time**. Being able to e-mail the report to an individual is.
I have a 'cgi-bin' directory on a unix server I can host script in.

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
