I need to create a form that will allow me to put in a customers check, bank information and then where signature is it will say "VERBALLY AUTHORIZED BY DEPOSITOR" The check will need to have all the standard fields to input; check number, customer information, routing number, account number, bank information, etc. I will then need to beable to print the check as a normal check. I have check forms that I will enter into my printer so i will need it to line up as a normal check similar to quickbooks.
