Virtuemart Attribute Customization

I need to duplicate this shopping cart....

Notes:

3 categories (Grand Rapids, Ann Arbor, Kalamazoo)

7 gift certificates per category (21 total)

I know how to use virtuemart but I don't know how to customize the checkout process. Here is the process I need...

1. View category (7 certificates)

2. Choose certificate (quantity box is displayed on details page. This is already standard in virtuemart)

3. Here is the part I need help with...I need to allow the customer (before checkout) to attach a separate mailing address and personal message.

4. Normal checkout process using PayPal. The separate shipping address should be visible on my receipt and there receipt.

I'm not sure if this is possible or if there is a better way of doing this. I will add the 3 categories and 7 products, I just need a virtuemart expert to add the "send this as a gift to a friend" area either on each product page separately so it's attached to the product itself (attribute) or program it into the checkout process.

I will deposit the winning bid into escrow. Please review the link above to get a better idea of how I need the checkout process to work.

I will provide access to FTP and Super Admin of Joomla backend.
