Hello, I am looking for a javascript/php developer to create a 1 page, landing page sign up form.

-with email validation (correct email address, mandatory fields)

-and preferably 1 collapsible/expandable section for the address (ie. "if you would like to receive a news mailer, check this box, and fill in your address here" (the address box expands when you select the checkbox)

-connects to a database, or to a flat file, csv, or xml file.

(the collapsible aspect is not as important but it is a nice to have).

The Information architecture - wireframe is done for this page,

-designed for a 1048 pixel wide screen.

-designing with tables is ok.

The form data will be sent either to a flat file, csv, or to a database.

The idea is to be able to parse the file for information from certain fields later on. say, all emails to friends, or parse all last names with letter. L. etc.
