Add Web Calendar to php Page

We are looking for a web calendar entry popup to our existing PHP form on our website.
Our existing form is at: [url removed, login to view]
We would like a similar pop up calendar entry system that is found on
[url removed, login to view] (when you click in the "Leave" or "Return" box) - that automatically fills in the dates as on that website.
This should be a simple project for someone that is javascript friendly.
Requirements:
1) Daily communication regarding project
2) Remove existing date script on the php form that is no longer needed
3) Add new calendar script to php form - there are 3 Date fields to be done
4) Keep existing php form clean and error free
5) Date should be displayed as xx/xx/xxxx, with current date highlighted and past dates can't be picked and should be grayed out.
6) The script should not run if the "Yes" option radio box is checked and should display "None" as it currently does now.
