IP telephony

I am new to this forum, I need an application similer to Hotfoon node softare, with which i can terminate calls to a local telephone line. I should have two small applications : one i will use abroad, the other will be on a computer in India with 24 hrs internet connection (Cable internet) and another telephone line connected with normal voice modem. When i place a call thru my caller program the call will reach the computer in India then it will dial the local telephone number then i should be able to talk to the person who pick the telephone line without any interuption.
