I am in the need of some **PHP/SQL** help. I have been working on a website called RolandConsulting.org. One of the features that this website is supposed to support for its users is a form that they can fill out online. This information will be used by Crystal Reports to be calculated and formated. The snag that I have run into would probably be minimal to an experienced coder. Here are some of the requirements:
1. I have hundreds of records containing information received from IRS files. These records can be sorted by Company Name and/or EIN Number.
2. I need a drop-down menu that pulls its information from within my table of IRS files in a mysql database. This dropdown menu needs to contain a list of all the organization names.
3. Once a person selects their organization name they would be able to see their IRS information and be asked to fill in the blanks and update the information with the most current year. However the original IRS data needs to be maintained.
4. Other questions would be asked and the answers would need to be stored in my database so crystal reports can draw from that data later.
I am using a site engine called
**PHP-Nuke** this engine supports user authentication. I need users who are signed in to my site to be able to update or change the information in the forms they submit. Users should only be able to change a form that they themselves submitted. The global user variables can be given upon request. ## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
