web form edits to paypal site

My site offers gift certificate purchases which are sent by email as a randomly generated code which they can use for purchases. The code can be entered during checkout on items for their discount. Currently the only option is to purchase one gift certificate at a time. I would like to make it so they have an option to buy more than one. 1. Need to add a field where customers can enter the quantity of gift certificates they want to purchase. 2. For these gift certificate purchases there needs to be a form they can fill out with their name, email, and mailing address. 3. When they submit the gift certificate order the admin and user will get the form data and the random GC code (they already get sent an email now as it is). Example: | x| Gift Certificates Name___________ Street___________ Town/City_________ Zip Code__________ Email____________ budget $45 MUST be available to complete it now. Listed before but everyone seems to be asleep when I try to contact them!
