We have a static web site that contains a contact page. This contact page currently contains a form with the following fields:
1 - Name
2 - Email
3 - Message
and the following options:
1 - Clear - This button simply reloads the page (i.e. clears the form)
2 - Send - Currently, this button opens up an email message.
We have the website hosted with [[url removed, login to view]]. As part of the hosting, GoDaddy provides the attached [url removed, login to view] script that will periodically poll for submissions and then email the data to my email address. They also provide a help page discussing how to use the PHP Form-Mailer.

We need someone to complete the following:
A - Modify the HTML to include the following fields:
1 - Name
2 - Company
3 - Email
4 - Message
B - Modify the HTML (excerpt is attached) and script so data is sent properly to my email (email accoutn is pre-defined within [url removed, login to view] website).
C - Modify the HTML and script so the user is prompted with an error if they do not enter any of the 4 items listed in requirement 'A' above.
If required, the hosting service also includes an MSSQL Database.
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. The work will be considered complete when information can be entered into the form and the data is sent to the predefined email address (already defined within the godaddy website).
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
