Appointment Booking Skill

I need a basic/simple appointment skill developed that collects 3 pieces of information (Name, Date, Time, Appointment Type) from the user and when completed submits the 4 captured slots/parameters to a JSON/Rest API that Is already built accepting these 4 parameters. Below are the details of the API to submit intent captured.
