In need of setting up a web form and an MS Access Database(.mdb) for a website (Windows Server) that will automatically be appended when a visitor submits a form.
What I'd want it to do is:
-email me a copy of the submitted form
-email the visitor a copy as well
-then append the database with the information
I'd rather keep everything in Access than using any MySQL backend options keeps it simpler for me.
• Student Name *required
• Student Birthday
• School Currently Attending
• Parent/Guardian Name *required
• Home Address – Street Address, City, State, Zip
(require at least one phone number but still give them the option to enter all three)
• Home Phone
• Work Phone
• Mobile Phone
