I need you to create a lookup table in ms.access.
Here is the current scenario:
users will enter information in the web (intranet), this form created in "Teleform", Teleform fields will be linked to access table, in which when user enters their emp.id. it should pull and pre-populate their name & site they are at. in the web (Teleform). then they select other fields which are :
Issue category ( few drop down selections, which i will give )
Urgency (few drop down selections which i will give)
and comments : with open box. for them to type out required info.
once they filled and hit send at the bottom of Teleform , it goes Teleform server and information should goto access table. the information they enter should goto to this new access table that you create.
I am stucked at access table, therfor need your expertise.

(if you don't understand the relationship between Teleform and [url removed, login to view],is also ok, you can just create a working access lookup table and access form and could be linked to our intranet as well) But if you know Teleform and how it works thats a advantage.
