I am writing a book (in many different medias) which will be a syndicated stories from many willing contributors. This book will be like "Chicken Soup For the Soul" type - a collection of stories from free contributors. I need an agreement for the contributors to sign.
Some contributors may choose to have a video interview or audio interview with me, some may choose to write their message and send to me.
I and my editor will choose appropriate content to be included in my publications, and we will edit them (without changing their message). I will have the video or audio transcribed to be included in the book. The video and audio will be made available online to the general public. So, they can choose to show their face or not show their face. They can choose to use their real name or other names.
They will not make claim to receive any rights or compensation from the publications. It is their free willing contribution to get their messages out.
They can fill out the form or request an appointment for an interview. I much welcome interviews For people who are willing to do an interview and be chosen in my publication, I will send them a copy of the book.
This contract must be universal. Because the people may come from many different countries.
This is what the form looks like. (a draft)

The easiest way to do this is to find a template used by publishing house and edit it.
You can use this attached document as reference. Or other legal templates to start.
