The user will click a button to launch the survey. The user will get a screen, which requires entry of their name and email address. They will click next. The system will send an authentication code to their email address for verification along with a unique identifier. They must enter this code on the next page and press verify. Once the code has been entered successfully, they are presented with the survey.
The survey will consist of 50 questions. The first question requires entry of a number from zero to four with two digits after the decimal point. The second question requires entry of a percentage from 0% to 100%. The remaining 48 questions are statements, followed by a 5-point likert-scale (strongly disagree = 1, disagree=2, neutral=3, agree=4, strongly agree=5).
When the user completes the survey, they click submit and the system will store in the MySQL database the user’s name, the unique identifier, their email address, date and time, the responses to all fifty questions and an index score. The index score is a number from 0 to 100 that the system calculates based on the answers to the fifty questions using an algorithm I can give you privately (it is not difficult to code).
The user is then sent an email with their index score and a series of recommendations based on the index score and the responses to all fifty questions. In other words, the email will need to be constructed programmably based on the survey responses using another algorithm.
The user may take this survey two or three times per year over the next four years and the system will need to store all records and not replace previous ones. (I am doing a time series research project).
