JotForm Patient Records Builder

I need a secure, well-structured JotForm set up to capture patient records for internal use within our healthcare team. The form should be intuitive for staff, yet comprehensive enough to collect all essential patient details (demographics, visit notes, attachments, consents, and any other fields we jointly confirm). Conditional logic will be useful so users only see fields relevant to the patient’s situation, reducing entry errors and speeding up workflow. Key requirements • Built inside JotForm, taking advantage of their HIPAA-compliant features (we already have a compliant plan). • Clean sectioning and clear labels so the form is easy to navigate on desktop and tablet. • Automated email notifications to designated team members once a submission is completed, plus an option to generate a PDF of the record. • Data validation on critical fields (e.g., DOB, phone, insurance ID) to maintain accuracy. • Simple dashboard or report view set up in JotForm Tables so administrators can sort and export data quickly. Acceptance criteria • Link to the live form, fully functional and tested. • All agreed fields present, with conditional logic working as intended. • Submission notifications and PDF output verified. • A brief hand-off document (or quick screen-share) showing how to edit fields or add users in the future.
