Financial Statement on My Account (Prestashop 1.5)

For Prestashop 1.5.6.1, I do require to include a new option on Front Office a section of "My Account", where for registered customers can review (and print) a financial statements.
Basically it is similar to a credit card balance but using order, vouchers & Credit Slips related with each customer's account.
There has to be a field to input start date, final date & a button to "Generate":
1. Initial Balance (of Orders to be delivered, Vouchers and Credit Slips).
2. A line per order with:
Order (Number, Product Code, Product Description, Date & Amount)
Vouchers applied (number of voucher and amount). There could be several vouchers for the same order.
Remaining balance of each voucher.
Note: Vouchers do have expiration based on current modules, so expiration (if it was not used during respective period) must be reflected as a separate line showing Voucher number, amount (not used), date of issue and date of expiration.
Credits Slips for the account and their use
3. Final Balance (detailing orders to be delivered, Vouchers balance & Credit Slips balance).
