I need a custom built data base that is driven by forms (READ ALL DESCRIPTION BEFORE RESPONDING)

I have a very specific job that I need. Please do not respond unless you read the entire description and can provide me with examples of your work that are similar to what I need. In basic terms, I want a dynamic “Primary Form” that contains “Sub Forms” within it. The “Primary Form” will be used by sales agents to report their sales results. They are sent to the agents each week and each agent receives a “Primary Form” that is unique to them. There might be 200 or more “Primary Forms” sent each week. The “Sub Forms” all have the same structure. There might be 1, or there might be 20 “Sub Forms” in the “Primary Form” The first field in the “Sub Form” is a drop down with multiple options to choose from. The second and third fields in the “Sub Form” are text fields that the user enters a number in to. Conditional logic is used to determine which of the second and third fields is presented, depending on the answer to the first field. The “Sub Forms” each have a unique name, which is determined by the item that is being sold by the agent. This item also has a unique code that is used when uploading the submitted data in to our main database. At the moment I have to manually add and name the “Sub Forms” to the “Primary Form” for each agent. I want that process to be automated so that I can simply upload an XLS or CSV file and the “Sub Forms” are created. After the agents submit their weekly “Primary Form” I want to be able to automatically download a XLS or CSV file for each agent that includes the results, numbers entered and the unique item code.
