The cell formats and the data validation lists in Excel say which fields are numeric, which are dropdowns and what ranges are allowed, so I would build the web validation from those rather than guess at them. That is why the sample file is the first thing I would ask for. Two things worth settling early. Any cell driven by a formula should be calculated on the server after submission rather than typed by the user. And a merged block in Excel is usually one field, not three, so the layout needs a quick pass before it becomes a form. On the point about adjusting fields later, I would define every field in a single configuration file: name, type, required, allowed range or option list. The form and the validation are both generated from that file, so adding a field afterwards is one entry and one column, not a code change. The hand-off guide would walk through exactly that. PostgreSQL is my recommendation. Setup is quick, it enforces mixed types and constraints properly, and it will not need replacing once the records build up.
