Must use Adobe Coldfusion MX 8 with Fusebox framework. Each company staff has an annual medical claim limit that reduces each time they visit registered clinics. The staffs account have their family members list with their pictures. There are 4 types of users; the Admin, User-Account Manager, Clinic Panel, company staff.
The Admin can monitor user logs and create user-accounts. The User-Account Manager can generate monthly or annual reports on claim usage and create [login to view URL] Clinic Panel can view the staff visiting their clinic remaining claim and deduct it respectively. Company staff can only view their profile and remaining claim value.
Staff list and some details are available in Access Database format. There are 3 to 4 forms (paper) as reference for the staff and clinic to fill-in to make their claim.
There must be at least 3 themes for the user interface to select during login.
