Need Analysis Document for USA Education Program

I am working on a program called "dual enrollment' which is an educational partnership between USA high schools and colleges. I am looking to help both high schools and colleges develop a system to analyze and evaluate their programs. I created a question and analysis document for high schools, this document is attached to this project, and as you see the purpose of htis document is to present 19 key questions to the high schools to get them thinking of the type of questions and analysis they shoudl be thinking about when evaluating their dual enrollment program. however, i only have one for high schools, i need a similar one for colleges. Some of the questions might be the same but many will be different since colleges have different objectives and purposes for their dual enrolment program than a high school might. please take a look at the two document attached and i need someone to create a questionnaire analysis document targeting colleges similar to the one i created for high schools
