Our company needs a peer-to-peer annonymous employee evaluation form (via website - that is password protected). Since it's annonymous, we won't need a login-id but we will want to send (email) a link to the form with a system generated random password to allow their access.
The evaluator will click the link, type the password and then see a drop down box of the list of employees (for his type - office or warehouse) that need to be evaluated (each employee has the opportunity to evaluate the other but not mandatory to evaluate all).
Upon selecting a particular employee, a blank evaluation (5 to 10 questions) form is presented, and the evaluator will record the scores. After successful submit, the just evaluated employee will be removed from the drop down box of that evaluator.
The evaluation is meant to be anonymous, so we don't need to store the individual employee scores based on the evaluator, but can store composite scores based on the questions and then use those to generate the various reports.
Also, we need some kind of an admin menu where employees can be added/removed, and the reports can be viewed (by the managers).
