A high percentage of the people of this Ruskopolis world makes a meager existence with low paying jobs and struggle to live at subsistence level. The group of healers of the world, control the money. They do so by charging high fees to the populous and by controlling the production of healing tools and the number of new healers entering the community. The healers have a strong group that represents them and advocates for them. The healers don't really know how to heal, unless it is by expensive testing and pills. They are only technicians with limited knowledge, but they hide this by the way all great dictators have done it, great publicity, image, image and image. They vest themselves in titles, dress in a "professional" and hang in their neck tools that are not of much use.
Once the aspiring healer enters the elite, he or she, has had it made. I does not matter what they do. They can diagnose BS and the world will follow their every word, they trust them. They cam "work" until they desire, and if they really screw up, chances are they will be forgiven.
The Ruskopolis populous on the other hand, have hard decisions. If they get sick and can not work, a lot of decide on suicide, because they can not afford the price of healers. The "insurance" set by the government has only allowed the healers to adapt conveniently and make every treatment expensive and unreachable.
This is only a short story without an end. There can be description on how the elite and how the populous lives, how the elite treats the populous. An old lady dies and the family sells her house and property to settle the debt with the healer. The healers participate in useless congresses, societies. etc that serve to maintain their status, image, they have a big stautes and paintings, a la Stanlin, to perpetuate their myth.
Be flexible, follow your creative uses, don't worry about words, bad words, don't really count. Good words can be brief and to the point.

excellent ideas, I am new to contests and just came back from a trip today. My mind is is in Timbuktu. Awarding twice, I get a chance of comparing two offerings, a lot of people don't submitt their articles, they are newbies also.
