Contest Brief

The page will feature a simple form in the centre of the page and should be responsive ready for mobile use. I will be doing the coding myself, but design is not my strong point.
The along with the page a logo mockup is also required. It will be titled 'Kitchin' and I was thinking simply a K for the logo itself but very open to ideas.
