Contest Brief

We have an employee detail and subcontractor detail form that we created using microsoft word that we would like set out professionally so that they are easy to complete and look professional. They are for internal use and will be filled out with pen so a PDF file would be fine.
Will mainly be printed on our office Minolta printer in A4 so need file to suit so it prints properly.
The information in the forms is as we would like however the layout is where we need some help.

Please Just Submit the Sub-Contractor Information form (single page) and we will select a winner before completing the second (4 page) form in the same format.
