I need a great design for a voucher that we can sell to customers so they can give it to their friends and colleagues as a gift, it needs to be one side, DL size and have some space to write the following details in manually:

Value
Expiry date
Voucher code for tracking/verification processing

I must look special and specific to our branding and use our logo which is attached, also, it must look great in both colour and black and white.

1- What is the exact desired dimensions?
2- Voucher code for tracking/verifications mean >> UPC code to scan. Right? If so, in which format you need your code?
