I need an award certificate for our little dental patients who are awarded a place in the "No Cavity Club". I would like to print 2 certificates per A4 sheet of paper. The background paper is metallic cream.
The certificate should say
No Cavity Club
It is hereby certified by
Riverside Dental Spa (Logo attached)
that our patient
Name----------------
is now a member of the "No Cavity Club" in recognition of their outstanding oral hygiene
