Konkurrence Instruktioner

I need a complete graphic design for a two-step signup page for my website campaign. The website is build in simple wordpress, and the campaign shall run seperately from that, most likely in clean html or php. See the attached two images as examples of how I want it to look.

The design should be complete in psd-files so that the text areas can be edited in different languages, and if special fonts is used these shall be delivered also. The design shall also be sliced in html ready for use with the insertion of form fields etc.

The book front image shall be designed as the "mockup template" shows it (only with one book though, not the three beneath it), where the book is illustrated seen as a real book slightly from the side. The book shall be inserted where the "popcorn/movie" image is in the attached image examples.

EDIT NOTE:

The design shall not be exactly the same as shown below, but a design similar. The one shown is in a movie-like theme, our design shall be in a "Sales" theme, but otherwise exactly like the example.
