Contest Brief

Looking to get a logo for our main crest on our new hockey jerseys. The team name is "Code Blue" I have attached 2 samples for guidelines of what we need.
1) team name needs to be on logo somewhere,
2) Hockey theme logo, ie) hockey sticks or pucks or something hockey related needs to be incorporated into design.
3) We are flexible on color combinations.
4) "Code Blue" (emergency code), is a hospital code used to indicate a patient requiring immediate resuscitation.
