I need new business cards created. I am looking for something different from the standard card. Please create something classy but unique

I have attached my logo and 2 images.

The card need

My Picture ( I will let you choose the photo to incorporate in card (you can slice as you see fit)
Name
Title
Address
Website
Email address
Phone Number

I uploaded 3 file, please make sure you see them all. two of them are psd files

On the back put a placemark for a QR code that I will provide and a slogan i will provide
