Contest Brief

Screenshots are a base form that converts badly , it needs some Cred. (CRO)
(Cred- Authority / gives the consumer confidence / sells the form / make the form more convincing)

"Customers come to this form to hire Earthmoving equipment or a Contractor, for example hiring a Bobcat operator to haul away some dirt from a landscaping project."

Alter/(add some of the below) to current design , so it Converts into a Lead (sale) this is a Lead Generating Form.

Ideas (maybe add /change?) testimonials, social proof, imagery that make things super obvious, industry/partner logos, header and or footer , links to main site / how it works/ about us, make T+Cs less dominant , highly convincing sales text

Supplied in PSD and PNG versions please. .
