I need forms designed for our Soccer packets that will be handed out at tryouts.

The CONTEST is only to design ONE PAGE of the packet. The winner will also do the rest of the forms in the same style as the winning entry. That way all the forms are uniform and look the same.

Please NOTE: There will be a few revisions to the numbers / wording as well as a few revisions needed so we
will have to stay in touch regarding additional forms.

Sample forms from last year are attached. I do not want them the exact same.
The logo will change on them as well.

Please design a new "GISA Slct cngrats 2013 final.pdf" form.
The older one is attached for example only.

The details of the new form are included in the word document called "Form Information 2014"
