Contest Brief

We are a membership program for business women. Women will pay to join our elite membership platform.
We require our form to be redesigned to become very sharp and professional. Very bank like.

We like the black and white colours. The form must be electronically filled. Please see our current version. Please add DOB to questions.

The design is to be very modern. We like using Playfair or didot font. Very Vogue style.

You can use images but they must be very modern and fashionable. Think Vogue or Marie Claire.

Women completing this form will be business owners and CEO's. Must be impressive easy to read.
