I have exported survey data from the results of a SurveyMonkey survey into an Excel spreadsheet and want to now put the questions up on my website in a format that looks like a self-service assessment tool.
I.e. I want the website user to see the questions, answer them, and then get a result that is calculated and weighted in accordance with my survey results. The end result should be a numerical score, say, out of 100. I don't want the user to see the weighting or the calculations, just the answers and the result and the ensuing interpretation (which I will supply). Should be pretty simple, I have attached the excel sheet for you.
I use Yola for my website, just in case it makes a difference to you. Your work needs to reflect my website's colour scheme and formatting.

Additional information:
Not only does it need to reflect the questions that were answered but it also needs to highlight the questions that were not.
Plus, depending on the questions answered there will be a score and an associated short report (which I will provide the words to) outlining the rationale behind the score. Then there is a call to action to contact me.
Can you do all that and make it work on Yola ?

As I said, I am using Yola as the back end of my website. I can input HTML code into the site, or append an external file.

Hi,

Just wanted a few clarifications, does your plan on Yola provide you PHP or ASP or any other kind programming language access? Because from the provided brief i get an idea that what you are asking for is a scoring system, and for the whole scoring system to be inaccessible to the viewer, you would require some sort of programming backend, as far as I know Yola does not provide this, I may be wrong, just asking.
